DENTAL CARE The Illinois State Dental Society matches volunteer dentists with patients for the purpose of providing dental care. Persons with limited income directly related to a disability, advanced age or a serious medical condition that restricts access to needed dental care may qualify. The Dental House Call program from the Illinois State Dental Society transports dental equipment directly to the home of patients who are homebound or in a care facility. Dentists charge for services but may offer discounts. Call (312) 440-8976.
FINANCIAL ASSISTANCE ContactWest Deerfield Township (847)945-0614 to determine your eligibility for financial assistance. Options include: General Assistance and Emergency Assistance are state-regulated functions of the Office of the Township Supervisor. Help may be granted to resident individuals requiring financial aid to assure basic life necessities such as shelter, food, utilities, drugs, health and safety. Low-Income Energy Assistance Program (LIHEAP) provides credits towards electric and/or gas bills. Circuit Breaker Grants provide low-income seniors and disabled residents with yearly grants to help pay property taxes, license plate fees and pharmaceutical expenses.
FOOD & NUTRITION Mobile Meals are available for delivery 5 days a week through Highland Park Hospital. Mobile Meals available for purchase include a hot lunch, plus a sandwich for dinner. EYECARE Seniors EyeCare services is available for adults 65+ who have not seen an ophthalmologist in three or more years and who do not have eye care insurance. Eligible persons may receive a comprehensive medical eye exam and up to one year of treatment for any condition diagnosed during the initial exam, with no out-of-pocket expenses. Call 800-222-EYES. Glaucoma EyeCare services is available for adults 65+ who have not seen as ophthalmologist in three or more years, are at increased risk for glaucoma and do not have eye care insurance. Eligible persons receive a glaucoma eye exam and the initiation of treatment if necessary. Call 800-222-EYES.

HANDICAPPED PERMITS The Deerfield Police Department providestemporary handicapped permits.Applications for a permanent handicapped placard are obtained through the Office of the Illinois Secretary of State.
HEALTH & WELLNESS The Lake County Health Center in Highland Park provides a wide variety of patient care, consultation & assessments for qualified residents. The Illinois Department of Public Aid offers financial assistance for Medicare premiums, deductibles as well as co-insurance to low income Medicare beneficiaries. The Patty Turner Center newsletter & website (www.deerfieldseniors.org) lists a variety of special interest fitness & wellness classes. Additional services include: 1) Free Blood Pressure/Blood Sugar Testing & low-cost Cholesterol screenings are conducted on a regular basis. 2) On-site podiatry appointments & massage therapy are available by appointment. 3) The Lending Closet offers the temporary use of wheelchairs, walkers, canes, bath/shower benches, commodes, raised toilet seats and other durable medical equipment. 4) The 50+ Lifestyle Expo, which takes place in October, offers flu shots, pneumonia vaccines, free health screenings, and access to a variety of vendors of older adult services. INDEPENDENT LIVING The Patty Turner Center can refer homebound seniors to resources that can help with the normal activities of independent living such as transportation, grocery shopping, social support and handyman chores (847) 432-4110.
MEDICAL BILLING & PAPERWORK ASSISTANCE SHIP volunteers at the Patty Turner Center 1) help interpret and resolve bills with medical bills, Medicare, Medicare Supplemental Insurance; 2) explain the Medicare supplements available: 3) review and explain long term care policies; and 4) explain the new Part D Medicare Prescription Drug Plan. PERSONAL & HOME SAFETY The Deerfield Police Department (Non-emergency: 847-945-8636) is available to answer questions related to home security, identity theft or internet safety.
The Deerfield/Bannockburn Fire Department (Non-emergency: 847-945-4066) offers free home inspections to assist seniors in assessing safety and eliminating home hazards. Assistance installing smoke/carbon monoxide alarms is also available. Blood pressure checks are available on the premises 8 a.m. - 4 p.m.
West Deerfield Township offers wellness calls to provide telephone reassurance to residents in need.
REAL ESTATE EXEMPTIONS Contact the West Deerfield Township Assessor’s office at (847)945-3020 to ensure you are receiving every exemption to which you are entitled.
Adults 65+ are eligible for the Senior Exemption in addition to the General Homestead Exemption.Tax Deferral Program allows Adults 65+ to defer payment of part or all of the property taxes on their homes. Assessment Freeze Homestead Exemption freezes the real estate assessment for eligible adults 65+.
REDUCED FEES for CITY SERVICES Adults 65+ who qualify under the federal HUD low-income rates are eligible for reduced rates of services such as garbage and water. The Village offers all adults 65+ a discount for vehicle stickers. For further information, call (847) 945-5000.
SOCIAL SERVICES Licensed social workers at the Patty Turner Center (847) 940-4010 and Deerfield Police Department (847) 945-5000 provide short-term counseling for issues related to eldercare, death & loss, aging parent/child relations, substance abuse, housing, etc. Home visits, support groups and referrals for counseling services are available as well as resources for residents in financial need.
TRANSPORTATION There are several transportation services available to Adults 65+ and handicapped or disabled residents: 1)West Deerfield Township paratransitvans provide transportation to and from medical appointments for mobility-challenged residents for a nominal fee. West Deerfield Township is also your link for 2)RTA Users Travel Cards that can be used for free travel on RTA-funded transportation. West Deerfield Township also provides a 3)Township Taxi Coupon Program for travel to and from Highland Park Hospital for seniors and disabled residents.4) Village of Deerfield Taxi Coupons can be applied towards travel within the Deerfield city limits. 5)Pace offers transportation for persons with disabilities. Upon approval, service may be requested 24 hours in advance. 6) The Patty Turner CenterProgram provides rides to medical appointments, stores, library, beauty salons, etc. by appointment.
